This is older but interesting. When ST elevation in lead III is greater than in lead II, this is a more sensitive finding than elevation in V4R in diagnosing right ventricular MI (RVMI). We can use this as a quick screening tool for RVMI.  Furthermore, III>II is a significant predictor of in-hospital mortality.
